Intractable migraine and migraine status by Neurologist Houston Randolph W. Evans, M.D.
People with migraine often seek help in the emergency
department, where they account for around 2% of visits and
may receive a less than warm welcome when they present
with “just a headache.”30 This challenging group of people
is made up of those with their fi rst or worst attacks, those
who have not taken or not responded to drugs, frequent
attendees, those with acute exacerbation of chronic migraine,
and those with migraine with neurologic symptoms and
signs (with aura). Intravenous fl uids and electrolyte replacement
may be necessary for patients with intractable vomiting
associated with migraine.
